Travel Consent Form is generally needed when a minor child travels internationally with only one parent or another adult (grandparents, older siblings, other family members, friends, etc.). Travel Consent Forms should be prepared in English, notarized by a local public notary, and receive an apostille before they can be translated into the language of the country of the child’s destination. The Travel Consent letter needs to include the following information: dates of travel, the full name and passport details of a consenting parent, the full names and passport details of the child and the person accompanying the child on this trip, the country of destination. We recommend you should check with your travel agent or an airline for their specific requirements and follow them precisely to avoid any problems during your child’s trip.

When having your Travel Consent Letter drafted and notarized make sure you use a Notary Public licensed in the state of your residency with at least five years of experience and thorough knowledge of the notarial laws of their state. Incorrectly notarized travel consent letters will not be accepted for apostille processing by the Secretary of State, resulting in travel delays and extra expenses. Please verify that your notarized Travel Consent Letter bears the notary’s signature, the notary affidavit, the date, and the notary seal. 

The travel consent form should be signed by the person giving permission and his/her signature should be notarized.  If both parents or legal guardians are permitting a minor child to travel outside of the USA, then both should sign it and their signatures must be notarized.
